DURING the 2007 crushing season a steam turbine at CSR Invicta Mill
suffered a major failure during operation. This paper covers the events
leading up to the incident and the subsequent identification of areas where
the management of turbines could be improved. A project team was
formed following this incident to develop processes that were
implemented at Invicta Mill, and then progressively rolled out to the other
six CSR Sugar factories to improve the management of steam turbines.
Details of processes developed in the areas of operator competency and
training, equipment reviews, risk assessment, maintenance,
documentation as well as reviews and auditing will be covered.
